Patient Participation Group

We are looking for patients registered at Hamilton Medical Centre like yourself who may like to join our Patient Participation Group.

The group of patients will meet together with Surgery staff at least once a year to discuss updates/changes/ideas/views that you may have for the practice.

This is also a chance for staff to answer any questions you may have regarding the surgery and the future of the NHS.

If you are interested but may not be able to attend in person then we will also keep the group informed via email if this is more convenient to you.

If you would like to join Hamilton Medical Centres PPG then please submit the form below.  If you have family and friends that are patients at Hamilton Medical Centre and would be interested in joining the Group then forms are available at reception.

If you require any more details then please contact the surgery.  Please can you send your completed forms back ASAP as we will be organising the next PPG meeting within the next couple of months.
